Patient Portal

Use the portal to access your chart, messages, and results (features depend on your visit and what the practice enables).

Portal accounts require an invite link sent by your practice (email or text).
If you do not have an invite link, call your preferred location and request portal registration.
If you see “No matching patient record found,” verify your name, date of birth, and phone number match what we have on file.

For privacy and security, portal access is controlled by the practice. If you need help, call the location you visit most often.
